Please book under your Aesthetic Practitioner. If your aesthetic practitioner does not provide a particular service, feel free to book with one of our other practitioners. See reason below to avoid appointment cancellations, or unexpected fees to your service.

Please note that all services are not interchangeable between providers, as each operate as an independent entity. If you have ongoing services, packages or gift cards with one aesthetic practitioner, they may not be accepted with another provider in the clinic. Please confirm at time of booking. Some circumstances will permit alternate practitioner booking, only upon advanced agreement of all parties. This will be explained on your first appointment. Thank-you!

Each Aesthetics Practitioner works diligently under their regulating body to provide safe, effective, beautiful treatments.

**When booking, select your Aesthetics Practitioner and then your treatment choice and keep in mind, each discipline provides different services. Please note services are not interchangeable from between Aesthetic Practitioners, and specials, gift cards, etc. may not be honored with a different provider (unless discussed in your consultation).** Call to book if you are unsure of how and who to book with.

GIFT CARDS purchased through an INDEPENDENT BUSINESS belong to that business. Please ask at the time of purchase how this applies, as each associate is an INDEPENDENT PRACTICE, working within Awad Medical Aesthetics & Wellness.

Modernized Menopause INTAKE FORM

Awad Medical Aesthetics & Wellness has revolutionized women’s wellness with MODERNIZED MENOPAUSE.

Offering highly effective treatments that are minimally invasive and positively impacts women’s health. We offer full menopause services: assessments, laboratory services, prescription and non-prescription treatments with follow up care!  

The MOST UNIQUE part of this program is this ONLINE INTAKE which gives access to menopause treatment to any woman in Ontario, Canada! No need to be seen in person at any point of your care. This modernized service eliminates potential gaps in menopause care.

HOW DOES IT WORK?

You do not need a Family Doctor or Nurse Practitioner. We are helping improve your access to care!

No referral is necessary. This is a self-referral.

Open to Ontario, Canada residents only. We will check identification after your initial intake.

Must be 35-65 years of age and born a biological female and have ovaries. We do not provide care to those who have had full hysterectomies, or trans-care hormone treatment, unfortunately as that is a specialty outside of our service offering.

Fees do apply** See below.

  1. Book your intake appointment (pick ANY space holder - it’s not a real appointment time or date - it’s simply a placeholder for us to see you have registered and we can check your forms.

  2. Pay the fee of $125 which includes access to the online intake form. Furthermore a follow up appointment within 72 hours of completion and all of your diagnoses, treatment recommendations, testing recommendations, prescriptions and instructions for follow up.

Services are offered by certified Nurse Practitioner NP) with additional certifications and specialty training in menopause treatment. Fees apply for your consultation, intake and follow up appointments only as NP services are not covered under OHIP billable services. However, if you have a valid OHIP card and 3rd party insurance benefits, you will not incur fees that are applicable to prescriptions or additional testing (if needed) such as blood work, or diagnostic imaging.
